Welcome to our online medical appointment self-scheduling platform, where you can easily schedule and manage physical and wellness exams, non-urgent problem visits, routine follow-ups, reproductive health visits (excluding prenatal), and vaccine appointments.

Online self scheduling is only available for medical appointments. Existing patients who have been seen in the last 3 years can use online scheduling to make a new appointment or change an existing appointment. If you are a new patient or are trying to book an appointment for an urgent medical need or prenatal care, please call us at 508-852-1805.

Sliding Fee Scale discounts for essential services are offered if you are eligible, depending upon family size and income. You may apply for a sliding fee discount at the front desk where you receive care. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
