Your Primary Care Provider (PCP)

A primary care provider (PCP) is a doctor, a physician assistant (PA), or a nurse practitioner (NP) who will oversee your care plan and is the leader of your medical home team.

Your primary care provider:
– Provides preventative care
– Identifies and treats common medical problems
– Assesses the urgency of your medical problems and directs you to the best place for treatment
– Makes and coordinates referrals to medical specialists, when necessary

Sliding Fee Scale discounts for essential services are offered if you are eligible, depending upon family size and income. You may apply for a sliding fee discount at the front desk where you receive care.
